New software tools are helping general radiologists diagnose lung diseases like experts, enabling earlier treatment and lower patient mortality. In this Imaging Wire Show interview, we talked to Jonathan Chung, MD, of UC San Diego about 4DMedical’s IQ-UIP software for identifying usual interstitial pneumonia.

All-Star AI for Prostate MRI

An AI model for prostate MRI that combines the best features of five separate algorithms helped radiologists diagnose clinically significant prostate cancer in a new study in JAMA Network Open. 

The Prostate Imaging-Cancer AI consortium was formed to address a nagging problem in prostate cancer screening.

  • Studies have shown that MRI can reduce biopsies and minimize workup of clinically insignificant disease, but it also has high inter-reader variability and requires a high level of expertise. 

The PI-CAI challenge brought together researchers from multiple countries with a single goal: develop an AI algorithm for prostate MRI that would improve radiologists’ performance.

  • Results were presented at RSNA and ECR conferences, as well as in a 2024 paper in Lancet Oncology that showed that individually the algorithms improved radiologist performance and generated fewer false positives.

But what if you combined the best of the PI-CAI algorithms into a single all-star AI model? 

  • Researchers did just that in the new study, combining the top five algorithms from the PI-CAI challenge into a single AI model in which each algorithm’s results were pooled to create an average detection map indicating the presence of prostate cancer. 

To test the new algorithm, 61 readers from 17 countries interpreted 360 prostate MRI scans with and without the model. 

  • Patients in the test cohort had a median age of 65 years and a median PSA level of 7.0 ng/mL; 34% were eventually diagnosed with clinically significant prostate cancer.

Results of PI-CAI-aided prostate MRI were as follows …

  • Radiologists using the algorithm had higher diagnostic performance than those who didn’t (AUROC=0.92 vs. 0.88).
  • PI-CAI working on its own had the highest performance (AUROC=0.95).
  • Sensitivity improved for cases rated as PI-RADS 3 or higher (97% vs. 94%).
  • Specificity also improved (50% vs. 48%).
  • AI assistance improved the performance of non-expert readers more than expert readers, with greater increases in sensitivity (3.7% vs. 1.5%) and specificity (4.3% vs. 2.8%).

The Takeaway

The new PC-CAI study is an important advance not only for prostate cancer diagnosis but also for the broader AI industry. It points to a future where multiple AI algorithms could be combined to tackle clinical challenges with better diagnostic performance than any model working alone.

  • AI for Thoracic Aneurysm on Breast MRI: Often called “silent killers,” thoracic aortic aneurysms are not as common as abdominal aortic aneurysms but are particularly life-threatening in women. In a new study in Nature, German researchers used a homegrown AI algorithm to screen for TAAs on breast MRI scans. In a trial with 5.1k women, AI detected 3.5X more aneurysms than radiologists (0.31% vs. 0.09%), showing that AI could be used for opportunistic TAA screening in women getting breast MRI.  
  • Bill Would Create More Residency Slots: More medical residency positions would be created – including for radiology – under a bill introduced into the U.S. House of Representatives last week. The Resident Physician Shortage Reduction Act of 2025 would address the physician workforce shortage by adding 14k residency slots over seven years. Despite consistent growth in healthcare utilization, the number of resident training slots has been capped by federal funding limits. The bill has already garnered the ACR’s support. 
  • Medical Groups Support AI Legislation: A broad coalition of 23 medical groups have lined up to support proposed U.S. legislation that would streamline Medicare reimbursement for AI software. The Health Tech Investment Act (S.1399) was introduced in April and would set up a dedicated Medicare coverage pathway for FDA-approved AI-enabled medical devices. Sources tell The Imaging Wire that while the legislation won’t be included in the big budget bill currently under consideration, it could be introduced in year-end legislation or in the next healthcare package Congress considers.
  • Canada to Reform Health Task Force: A Canadian health task force that wanted to keep the country’s mammography starting age at 50 will be reformed and modernized under a new proposal by the federal government. The Canadian Task Force on Preventive Health Care was criticized after it declined to follow the USPSTF’s lead in lowering the age to 40, prompting the government to pause the group and start an external review. The report was published last week and includes a number of recommendations.
  • Short Videos Boosts CT Lung Screening: To boost participation in CT lung cancer screening, more aggressive patient outreach is needed – especially for people living in economically disadvantaged areas. Researchers writing in JACR described how they developed short, one-minute videos featuring radiologists and patients telling stories about screening’s importance, and 636 Medicaid patients due for screening were notified of the videos through email and patient portals. Those who watched the videos were more likely to schedule or complete screening at 12 months (32% vs. 23%).  
  • HAP Scores NH Revenue Cycle Contract: Healthcare Administrative Partners scored a contract to provide revenue cycle management services to Nashua Radiology in Nashua, New Hampshire. HAP will manage billing, coding, carrier credentialing, business intelligence, and MIPS measure assurance services. The contract adds to HAP’s growing roster of clients in New England. 
  • CT Falls Short for Determining Brain Death: Imaging alone isn’t accurate enough to replace clinical assessment for determining brain death in patients with severe neurological injuries. In a new study in JAMA Neurology, researchers used CT perfusion and CT angiography in 282 patients to see if they could be used to assess death by neurologic criteria, defined as permanent cessation of brain activity. But neither modality achieved the accuracy threshold of 98% that’s required to support their use as a standalone test. Instead, imaging should support bedside clinical assessment.
  • Viz.ai Gets FDA Nod for Brain CT AI: Viz.ai received FDA clearance for Viz Subdural Plus, an AI algorithm that analyzes non-contrast CT scans to quantify fluid buildup from conditions like subdural hemorrhage. The algorithm helps clinicians by automatically calculating volume and maximum thickness to determine SDH severity and guide treatment. Separately, Viz touted a case study showing how its Viz ICH and Viz ICH Plus algorithms improved management of intracerebral hemorrhage by cutting interhospital transfer time in half (200 to 101 minutes). 
  • PET Finds Nasopharyngeal Cancer Biomarker: Researchers from China used PET/CT to identify a biomarker that can be used to identify nasopharyngeal cancer, often caused by the Epstein-Barr virus. Writing in a study in JNM, they used the fluorine-18 RCCB6 radiopharmaceutical in 25 patients to detect CD70, which is expressed by nasopharyngeal cancers. Uptake of F-18 RCCB6 on PET/CT correlated well with CD70 expression and helped identify primary tumors and local lymph node metastasis with high sensitivity (100% and 93%, respectively).
  • GE Adds to MIM Software: GE HealthCare has added its own proprietary features and algorithms to MIM Encore, the PET and SPECT viewer it acquired through its purchase of MIM Software in 2024. MIM Encore now features dedicated post-processing tools for GE PET/CT and SPECT/CT systems, including resolution recovery and motion correction for SPECT oncology, support for new cardiology radiotracers and algorithms, and 3D rendering and advanced correction for neurology.
  • AI of Chest X-Ray, Mammo Predicts Cardiac Risk: A deep learning algorithm that analyzed mammograms and two-view chest X-rays predicted 10-year cardiovascular risk in women ages 40-60 better than traditional ASCVD risk scores. In an award-winning paper presented at SIIM 2025, Emory University researchers found their in-house algorithm had higher hazard ratios than ASCVD scores (HR=1.02) for calculating breast arterial calcification from mammograms (HR=1.14) and embeddings (numerical representations) from frontal and lateral chest X-rays (HR=4.91 and 2.37 respectively).
  • Lunit Touts Hajj Season Screening: Lunit is highlighting the use of its Insight CXR chest X-ray AI algorithm during this month’s Hajj season in Saudi Arabia, where it assisted Saudi health officials in screening pilgrims for chest diseases like tuberculosis and pneumonia. Hundreds of thousands of chest X-rays were acquired over just a few days, underscoring the ability of AI solutions like Insight CXR to efficiently screen and triage large populations. 
  • U.K. Radiologist Killed in Plane Crash: A U.K. radiologist was among the over 240 people killed in last week’s crash of an Air India flight. Dr. Prateek Joshi was a radiologist at Royal Derby Hospital and was traveling back to the U.K. with his wife and three children. A BBC report said he moved from India to Derby in 2021 and worked at the hospital for four years. Investigators are still trying to determine the cause of the crash.
