The American College of Cardiology’s annual meeting is wrapping up today in Chicago, and new research into coronary artery calcium scoring has been one of cardiac imaging’s top trends at McCormick Place.
CAC scoring has been around for ages as a way to detect and quantify calcium buildup in the coronary arteries based on data from non-contrast CT scans.
- But it’s only been in recent years that CAC scoring has come into its own as a tool for predicting risk of mortality and major cardiac events – in some cases years before they happen.
Clinicians are learning that they can use CT-generated CAC scores to estimate future risk and guide interventions to reduce it, such by prescribing statins or behavior modifications.
Research presented at ACC 2025 underscored CAC scoring’s potential …
- In the CLARIFY CAC screening program, researchers found a 6.2% rate of thoracic aneurysm, indicating a need for screening and prevention.
- CAC scores of 0 were more common in women than men (49% vs. 23%), but there was no statistically significant difference in non-calcified plaque rates between genders.
- Researchers found moderate accuracy (AUC range=0.60-0.73) for a method of generating CAC scores from 12-lead ECG data rather than non-contrast CT scans.
- Bunkerhill Health’s I-CAC algorithm was used to generate automated CAC scores for 200 patients. After six months, patients with scores >400 had a 17% rate of cardiac events and 11% all-cause mortality.
- A commonly used measure for low-value care based on administrative claims classified too many CAC tests as inappropriate, with a positive predictive value of only 43%.
- A case study focused on the paradox of a 59-year-old healthy triathlete with a CAC score of 780, possibly due to chronic coronary stress from high-endurance exercise. Invasive testing was deferred in favor of medical therapy due to his low cardiac risk.
- On the other hand, a literature review of 19.4k people found no statistically significant difference in CAC scores between endurance athletes and healthy controls.
- Non-calcified plaque in patients with CAC scores of 0 was common (26%) in residents of rural Appalachia, indicating high risk of rupture and suggesting the limitation of relying on CAC scores.
- A Sunday debate discussed whether CAC scoring should be added to mammography and colon cancer screening, or reserved as a decision aid.
The Takeaway
The studies from ACC 2025 show that CAC scoring has a bright future – bright enough that it’s generating heightened interest from cardiology. New CAC scoring tools arriving on the market should improve its predictive value even more.

- AI for DBT Mammography: Most recent research studies on breast screening AI have focused on 2D mammography. But a new arXiv preprint publication from Emory describes use of Lunit’s Insight DBT algorithm in 163.4k screening 3D mammograms. The algorithm had overall good performance (AUC=0.91) with “robust” performance across age, race, and ethnicity, although there was a slightly lower AUC for heterogeneously and extremely dense breasts (0.91 and 0.87). Performance was also lower for noninvasive cancers and calcifications.
- ACR Disputes BMJ Study: The ACR is disputing findings in a meta-analysis published in BMJ on interventional radiology-based treatment for chronic spine pain. The February 19 article included a strong recommendation against interventional procedures like spinal injections and radio-frequency ablations, claiming the treatments are “not curative” and are “costly.” That’s disputed by ACR and other specialty societies, who questioned the paper’s methodology and conclusions. They urged BMJ to retract the paper.
- GE Launches New Invenia ABUS: GE HealthCare launched Invenia ABUS Premium, the latest version of its automated breast ultrasound technology. First previewed at ECR 2025, the scanner includes new features powered by GE’s Verisound AI technology, such as scan quality assessment for proper breast coverage and positioning, auto nipple detection for consistent marker positions, and a fast scan tool that boosts scan speed 40%. The scanner’s viewer includes an AI assistant feature for making third-party AI tools available to users.
- GE Debuts One-Beat Cardiac CT: Another system GE HealthCare teased at ECR 2025 that it’s now showing to U.S. audiences is Revolution Vibe, a new CT scanner designed to enable hospitals to get into cardiac CT at a lower price point than previous high-end offerings. Vibe performs unlimited one-beat cardiac imaging and supports full-heart images acquired at low radiation dose, with GE’s TrueFidelity DL and SnapShot Freeze 2 technologies for higher image quality without motion artifacts. Revolution Vibe is CE Marked and FDA 510(k) clearance is pending.
- Guerbet Hits AI Milestones: The AI program at French contrast developer Guerbet achieved several recent milestones. Guerbet received the CE Mark for its DUOnco Bone solution for detecting and localizing bone lesions on thorax-abdomen-pelvis CT scans. Meanwhile, the FDA gave Guerbet’s DUOnco Pancreas algorithm for analyzing CT pancreas studies “breakthrough device” status, as there are no FDA-approved solutions for detecting incidental pancreatic cancer. Both applications are being developed by Intransense, in which Guerbet has a 39% stake.
- Clinical Trial Firms Unite: Clinical trial data provider Clario acquired NeuroRx, a developer of imaging analysis solutions for multiple sclerosis research. NeuroRx supports drug development trials with imaging-related services like protocol assistance and image interpretation and reporting, and has supported the approval of 12 MS drugs since 2011. The acquisition will expand NeuroRx’s reach and broaden its impact on drug development.
- Radiopharm Touts Theranostics Results: Theranostic radiopharmaceutical developer Radiopharm Theranostics is highlighting data supporting its gallium-68-based 68Ga-RAD202 tracer presented at the European Molecular Imaging Meeting 2025. German researchers found that 68Ga-RAD202 binds specifically to the HER2 receptors in HER2-positive xenografts, with a high tumor-to-background ratio. Follow-up treatment with a companion 177Lu-RAD202 therapeutic agent was well-tolerated and demonstrated prolonged survival time.
- KA Imaging Gets Premier Contract: Digital X-ray developer KA Imaging was awarded a national group purchasing agreement and technology breakthrough designation from Premier for its Reveal Mobi Pro system. The designations will make it easier for Premier members to acquire the mobile X-ray system, which is outfitted with KA’s Reveal 35C digital detector and can acquire conventional and dual-energy images at patient bedside with a single image acquisition.
- Brain Scanner Takes to the Skies: You’ve heard of scanners installed in ambulances, but how about airplanes? EMVision Medical Devices reported a successful proof-of-concept test of its First Responder PoC scanner installed in an airplane operated by Australia’s Royal Flying Doctor Service. The helmet-sized scanner could help diagnose patients with stroke and TBI before they reach the hospital, speeding triage and diagnosis. The electromagnetic imaging scanner acquired remote scans of volunteers and next will be tested with suspected stroke patients.
- AngioInsight Touts Angio AI Study: Cardiovascular AI developer AngioInsight launched the SMARTFLOW clinical study, a pivotal trial to investigate the accuracy and effectiveness of its AngioAI+ platform. AngioAI+ uses AI and machine learning to improve angiography image interpretation, providing 3D reconstructions, noninvasive FFR estimations, and other calculations to diagnose coronary microvascular disease. Data will be used to support an FDA application for the technology.
- Imaging Diagnoses Belly Dancer’s Dyskinesia: A case report in JAMA Neurology details imaging’s role in diagnosing a 53-year-old man with a rare condition: belly dancer’s dyskinesia. He presented with slurred speech, right lower limb weakness, and “spontaneous, arrhythmic, spasmodic contractions of the abdominal muscles.” DWI-MRI scans revealed acute infarcts in the corpus callosum and left frontal and parietal lobes, while CT showed severe stenosis in the left anterior cerebral artery. After treatment with antiplatelet agents his abdominal fluttering resolved in 24 hours.
