Cardiac Imaging in 2040

What will cardiac imaging look like in 2040? It will be more automated and preventive, and CT will continue to play a major – and growing – role.

That’s according to an April 11 article in Radiology in which Dr. David Bluemke and Dr. João Lima look into the future and offer a top 10 list of major developments in cardiovascular imaging in 2040.

Cardiovascular disease carries a massive medical burden, with over 800,000 myocardial infarctions occurring annually in the US alone. By 2030 almost one-third of deaths worldwide are expected to be due to cardiovascular disease.

Multiple different imaging modalities are adept at identifying both ischemic and nonischemic heart disease, but CT has risen to the top for ischemic imaging, making “quantum” advances in the last decade thanks to its growing prowess in the coronary arteries.

CT’s advances have been so great that the modality occupies seven of the top 10 spots on Bluemke and Lima’s list. In brief, they see: 

• Coronary CTA becoming totally automated, a development that will no doubt benefit AI developers like HeartFlow (see below).
• CCTA becoming a preventive tool rather than a gatekeeper to interventional cardiology (also hinted at in a recent study from Denmark). For example, CCTA will be used to track the effectiveness of statin therapy
• Photon-counting CT flexing its muscles for coronary artery evaluation and routine plaque characterization and quantification
• Next-generation cardiac CT becoming more like MRI
• Next-generation cardiac MRI becoming more like CT

They also see a major growing role for software-assisted cardiac CT with AI and other tools. Software-based automation has simplified the “postprocessing nightmares” once common with coronary CT, making it “wonderfully ordinary” to perform. 

The Takeaway

Bluemke and Lima offer a fascinating glimpse of cardiac imaging’s future. But one area they don’t touch on is whether CT’s rising prominence means radiologists will start taking back turf in heart imaging once ceded to cardiologists. Heart specialists haven’t taken over cardiac CT in the same way that they monopolized echocardiography and nuclear cardiology. Could we be seeing a renaissance of radiology in the heart?

• Women Need More Radiation Protection: Women who work in healthcare close to sources of ionizing radiation need better protection in the chest area, particularly women who work with image-guided procedures, say researchers in an editorial in The BMJ. They cite studies showing higher breast cancer rates in female physicians who work with radiation, and call for additional protection to be worn under standard gowns. The article highlights the challenge of attracting more women to interventional radiology, where there is already a gender gap.

• Volpara Signs Karmanos: Volpara Health will be installing its Risk Pathways risk assessment and patient management software at McLaren Health Care’s Barbara Ann Karmanos Cancer Institute. The software will assess the breast cancer risk profiles of patients getting screening and diagnostic mammograms at Karmanos to enable specialists to triage high-risk patients for follow-up care, including genetic testing or additional imaging. Breast care providers are focusing on risk profiles as a way to identify high-risk women earlier. 

• DBT Beats Digital Mammo: In a massive study in Radiology of nearly 900k women and 2.3M screening exams from 2011 to 2018, DBT showed its superiority over conventional digital mammography for breast screening. Researchers found that DBT was superior on most radiologist performance metrics, and could contribute to better long-term screening outcomes. Some 84% of US sites were using DBT as of September 2022; this study confirms previous findings that DBT is now breast screening’s gold standard.

• CMS Inpatient Hike Falls Flat: CMS is proposing a 2.8% increase in payment rates in 2024 for the Inpatient Prospective Payment System, which covers Medicare reimbursement for inpatient hospital care. CMS says this increases hospital payments by $3.3 billion, but the American Hospital Association called it “woefully inadequate” considering the economic pressures hospitals are facing (inflation was just reported at 5% for March) and the fact that half of hospitals finished 2022 with a negative operating margin.

• Blood Pressure’s Link to Dementia: Researchers used MRI to find a link between early-life hypertension and brain health later in life. In JAMA Network Open, researchers used 3-tesla MRI to scan 427 adults older than 50; those who had hypertension when they were young tended to have lower regional brain volumes and poorer white matter integrity. The findings are in line with other studies showing that modifiable risk factors early in life can lead to Alzheimer’s disease and other dementias.

• PE Still Loves Healthcare: Despite ongoing turmoil in the rest of the investing market, private equity continues to love investment opportunities in healthcare. Bain & Company’s new report on PE and M&A activity in healthcare says that 2022 was the second-best year on record for healthcare investing, with $90B in deals getting done. That’s way down from $151B in 2021, but still $10B more than the next-closest year. Generative AI and healthcare IT remain hot sectors. 

• HeartFlow Lands $215M: Perhaps no story this week exemplifies healthcare’s attraction to investors more than HeartFlow’s massive $215M fundraising round. The company has carved out a leadership position for its FFRCT software, and it plans to use the new funds to further expand in the CCTA AI segment. HeartFlow already has a head start on its competitors, and the new funds may help it extend its lead.

• Annalise Nabs FDA Nods: Annalise.ai has expanded its portfolio of AI triage and notification applications with new FDA clearances for seven additional findings. The clearances apply to the company’s Enterprise CXR chest X-ray and Enterprise CTB non-contrast brain CT applications; the CT clearances mark the company’s entry into the US neuroimaging segment and build on the global launch of its Enterprise CTB in October 2022. Triage and notification are becoming key sectors of the AI market. 

• Hyland Lays Off 20%: Hyland has laid off 1k employees, or about 20% of its workforce, in a restructuring that also impacted its healthcare IT division. In an open letter to employees, Hyland President and CEO Bill Priemer said the company has been impacted by inflation, rising interest rates, and wage increases, while many customers have been pulling back on technology purchases due to the economy. Hyland in recent years has acquired a portfolio of radiology IT companies that include Acuo, PACSGear, and other firms. 

• Riverain Deploys at Cedars-Sinai: Riverain Technologies is deploying its ClearRead CT software at Cedars-Sinai for use in the lung health program at the hospital’s inpatient and outpatient facilities. The application uses Riverain’s Clear Visual Intelligence technology to see past structures like bone and vessels to provide a clearer view of lung nodules; studies have shown it can reduce missed nodules by 29% and nodule search time by 26%. Cedars-Sinai has been using Riverain’s ClearRead Xray software since 2012.

• 4D Medical Scores US SaaS Site: Australian digital x-ray developer 4D Medical has scored the University of Miami as the first US site for its SaaS-based XV Technology for assessing lung ventilation. 4D will deploy its XV Technology at the university, and will process patient data on demand with its Lung Ventilation Analysis Software, providing LVAS ventilation reports back to the university. 4D debuted its XV Scanner as a work-in-progress at RSNA 2022.
