The funniest physician on the internet, ophthalmologist and comedian Dr. Glaucomflecken, sparked quite a debate over private equity’s healthcare impact last week with this banger of a Valentine’s Day tweet:
“Every physician who sells their practice to private equity is choosing to make health care worse for everybody. I hope the money helps you sleep at night, because you have made life worse for every single patient and employee walking into your PE Daddy’s practice.”
Within three days, Dr. Glaucomflecken’s attack on healthcare PE garnered 1.2M views, 1,150 retweets, and 12k likes, while inspiring some telling conversations about private equity’s impact on radiology.
RadTwitter’s many private equity critics…
- Celebrated one of their biggest concerns gaining viral attention
- Warned that this trend is putting MBAs in control of patient care
- Theorized that PE is “driving physician satisfaction into the ground”
- Highlighted PE-backed rad practices’ staffing/retention challenges
- Joked that Dr. Glaucomflecken is now uninvited from the ACR meeting
Meanwhile, a few brave radiology PE leaders and defenders….
- Countered that Dr. Glaucomflecken’s post was unfairly broad
- Emphasized the challenges that private practices face on their own
- Reasoned that health systems just are as money-driven, and worse at leading practices
- Contended that PE improves radiology access in rural areas (others disputed this)
- Inferred that PE is “in the arena” working to improve care, while critics sit on the sidelines
The hundreds of other comments from non-radiologists in the Dr. Glaucomflecken thread made many of the same arguments about their specialties, while revealing an overall consensus that the healthcare incentive system is flawed, insurer influence is playing a big role in practice consolidation, and many physician practices aren’t in a position to exclusively sell to physician owned/led organizations.
The Takeaway
Regardless where you stand in the healthcare private equity debate, Dr. Glaucomflecken’s Twitter responses make it very clear that providers are concerned about the state of U.S. healthcare economics. That same discussion thread also might contain more ideas about areas that the U.S. healthcare system should improve than any published report we’ll cover this year.

Annalise.ai Gets ‘Comprehensive’ with Enterprise CTB
Annalise.ai doubled-down on its comprehensive AI strategy with the launch of its Annalise Enterprise CTB solution, which identifies a whopping 130 different non-contrast brain CT findings. Annalise Enterprise CTB analyzes brain CTs as they are acquired, prioritizes urgent cases, and provides radiologists with details on each finding (types, locations, likelihood).

- Siemens & Oncimmune’s Multimodal Alliance: Siemens Healthineers and cancer blood testing startup Oncimmune will work together to evaluate their combined technologies’ ability to assess lung nodule malignancy, and potentially reduce unnecessary biopsies. The companies will first combine Oncimmune’s EarlyCDT Lung blood test with Siemens Healthineers’ AI-Rad Companion Chest CT solution for a validation study, which might lead to future commercial collaborations.
- Rad+SLP MBSS Reporting: A University Hospitals team developed a more efficient reporting structure for modified barium swallow studies (MBSS) to diagnose dysphagia, which combines radiologist and speech language pathologist reports (vs. separate interpretations and reports). The team successfully implemented the integrated MBSS reporting workflow and confirmed its efficiency improvements, but noted that they had to overcome challenges along the way (CMS only reimburses separate reports, lack of training).
- Hospital CEOs Top Concerns: A survey of 281 hospital CEOs revealed that workforce challenges were their top concern for the second year in a row, beating out financial challenges and behavioral health issues. Financial challenges previously ranked as the top concern for 16 consecutive years before workforce woes took over, but CEOs are now battling shortages of registered nurses (90%), shortages of technicians (83%), and burnout among non-physician staff (80%).
- Bionic Finger Imaging: A China-based engineering team developed a “smart bionic finger” that uses subsurface tactile tomography to capture a series of image slices and create 3D profiles of anatomic structures – just by touching human skin. The authors envision this technology being used by bionic robots, either to assist doctors or for non-medical applications.
- Dynamic Radiography for Heart Failure: A new EJR study highlighted dynamic chest radiography’s (DCR) potential to serve as a less invasive hemodynamic imaging method for heart failure patients. The study compared 20 HF patients’ DCR exams and right heart catheterization exams, finding high correlations across some key hemodynamic parameters (not all), and showing that DCR was able to identify patients with a low cardiac index (AUC: 0.792) and patients with low cardiac index and high pulmonary artery wedge pressure (AUC: 0.902).
- Radiographer Back Pain: New research out of Ireland suggests that radiographers are 4.7-times more likely to experience low back pain (LBP) than the general population. The 151-radiographer survey revealed that 50% of respondents currently have LBP and 75% experienced LBP within the previous 12 months, forcing them to reduce work and/or leisure activity (25% & 43%).
- CCTA Radiomics Plaque Predictions: A new Radiology study showed that CCTA-based radiomic signatures of coronary plaque can identify vulnerable plaques associated with higher future risks of adverse cardiac events. Researchers in China developed and validated the radiomic signature using two CCTA datasets (225 & 708 patients), detecting vulnerable plaques in an external dataset with “moderate to good” performance (AUC: 0.77), and showing that a high radiomic signature (≥ 1.07) was independently linked to MACE over 3-years (HR: 2.01).
- Eagles Malpractice Verdict: Former Philadelphia Eagles safety Chris Maragos was awarded $43.5M in a medical malpractice case that largely focused on the diagnosis of his career-ending knee injury. Maragos was injured in October 2017 and underwent surgery for a torn PCL, although a May 2018 MRI revealed further deterioration and a persistent partial tear, and both exams potentially missed a meniscus tear. The team captain was nonetheless cleared to increase his rehab activities (including running), causing further damage to his knee.
- Missed Aortic Aneurysm Screenings: The vast majority of privately insured patients who qualify for abdominal aortic aneurysm screenings don’t receive their recommended exams. A new JVS study analyzed 2006-2017 data from 35k eligible patients (male ever-smokers, 65-75yrs), finding that just 13.6k / 38.7% underwent the ultrasound-based screening. That’s well below the 74-79% adherence rates that have been reported by integrated health systems (the VA, Kaiser) and suggests that interventions are required to address this shortcoming.
- Physician Turnover AI: A Yale-developed AI algorithm was able to predict physician turnover based on their EHR data with 97% accuracy, findings that could help health systems intervene before physicians decide to leave. Although there was room for improvement in the model’s sensitivity and specificity (64%, 79%), the AI gave good insight into the top variables contributing to turnover risk: physician length of employment, their age, the complexity of their cases, and the demand for their services.
